Resolution of inflammation on mucosal surfaces is a critical process for IBD patients. GWAS revealed SNP variants within the gene encoding gasdermin-B (GSDMB) that are associated with increased susceptibility to IBD and elevated expression of GSDMB. The objective of this study was to determine the expression pattern and the importance of the role of GSDMB in the pathogenesis of IBD. Primary intestinal biopsy and isolated intestinal epithelial samples from IBD patients and non-IBD controls were evaluated for mRNA and protein expression.

Using the human intestinal epithelial cell line, HT-29, CRISPR-cas9 knockout cells lacking GSDMB were generated and used for in vitro assays for TXT, cell death, and cell migration. In addition, human epithelial cells transfected with GSDMB constructs and/or stimulated with methotrexate (MTX) were used for relevant functional assays. We report for the first time a dramatic increase in full-length epithelium-specific GSDMB in IBD patients compared to healthy controls, who, unlike other members of the gasdermin family, are not involved in program-mediated cell death.

Furthermore, we describe a functional decrease in proliferative activity in vitro, wound repair, and an increase in focal adhesion proteins in epithelial cells lacking GSDMB, phenotypically rescued by transfection or MTX treatment. Importantly, GSDMB expression is decreased in IBD patients in remission, to the levels of controls without IBD. Taken together, our data suggest that the epithelium-derived increase in GSDMB during IBD has a protective role and promotes specific epithelium adhesion and migration, justifying the potential therapeutic use of GSDMB to optimize intestinal mucosal healing and restoration of homeostasis.
